Swimmers

Each swimmer is required to have a completed LBSL Insurance form for the first day of practice, children without this form will not be permitted into the pool.   The form can be downloaded here

Divers

Anyone diver is also required to have the LBSL Insurance form as well as a USA Diving Limited Athlete Membership which includes insurance.  This process is done online and when completed you will be able to print out a membership card, PLEASE do so. The original cards take 6-8 weeks to receive.  Sign up here.
